Guidelines for Consideration: Requests must be submitted in writing, 501(c)(3) Tax Exempt status, Benefits citizens where our customers live & work

Providers/Hospitals^*

*Joint sponsorships with hospitals and providers for programs that benefit the community are encouraged. Direct support to hospitals and providers is prohibited.

Note: Because Anthem Blue Cross and Blue Shield is a major supporter of the United Way, we donate funds to United Way member agencies primarily through our annual associate and corporate campaign, rather than directly to member agencies.

Our Awards Process: Organizations should submit sufficient information to enable the reviewers to evaluate the merits of the proposals. Concise, clearly written proposals are preferred. Proposals should include:
Brief description of the organization
Brief description of the program to be funded
Rationale for amount requested
Copy of IRS 501(c)(3) tax exemption letter
Description of how the program benefits the community
Explanation of how the program's success will be evaluated

Proposals should be submitted at least 60 days prior to the date the funds are needed.
